A very good documentary has made by BBC Television. It says that if we obeyed the rules of the risk analysis on time, the disaster couldn’t have happened.

3. Mitigate: The project team acts to reduce the probability of the occurrence or impact of a risk.
4. Accept: The project team decides to acknowledge the risk and not take any action unless the risk occurs.
